Wound bed preparation

The concept of “wound bed preparation” can be defined as the overall wound management procedure aimed at accelerating endogenous healing and increasing the effectiveness of technologically advanced products used for care. The ultimate goal is to ensure the formation of healthy granulation tissue until complete closure.

Wound Bed Preparation can be broken down into three phases:

      • Debridement

      • Control of the bacterial charge

      • Management of exudate.

These are the processes that prepare the wound bed to face all the events necessary for healing: therefore, for therapeutic success, it is very important that each operator fully understands how and why these local barriers must be removed.

Biofilm

Infections connected to biofilm represent a serious problem because they reduce the effectiveness of therapeutic treatments, the response capacity of the immune system and consequently can increase the mortality related to infections or favor their chronicization.

It is estimated that 60% of 100% of chronic wounds contain biofilms:

  • • The presence of biofilms can lead to a chronic inflammatory stage
  • • The immune system tries to fight the biofilm by releasing neutrophils and macrophages which release reactive oxygen species (ROS) and proteases (MMPs)
  • • ROS and proteases can too damage tissue health and impair wound healing
